Students must have a pass from their teacher to come to the clinic.Students should call their parents from the clinic when they are ill, not from their teachers classrooms.All students are required to sign out in the attendance office when going home ill, with a dismissal slip from the clinic.

All prescription and non-prescription medication must be kept in the clinic with the appropriate forms.

Physician's Statement for Administration of Prescription Medication must be signed by the prescribing Physician and the parent/guardian. Prescription medication must be provided in the original container appropriately labeled with dosage and instructions.<<FORM>>Non-prescription medication may be administered when Parent's Statement for Administration of Non-Prescription Medication is filled out and signed by the parent/guardian. Non-prescription medication must be provided by the parent/guardian in the original container with the original label of instructions.<<FORM>>

Inhalers may be carried by students IF the Physician's Statement for Student Held Inhaler form is signed by the prescribing Physician, parent/guardian and the student. This form must be turned into the clinic and/or athletic trainer.<<FORM>>
