All girls interested in trying out for Girls Soccer must have all paperwork on file ASAP. We are currently accepting paper work for the 2013-2014 soccer season.
All Physicals MUST be dated after April 1, 2013 and on the proper form to be accepted.
Your athlete will need the following in order to partcipate in the soccer PE class or to try-out for the team in November.

(3)SO-1-consent to treat
(4) SO-1 (pg2)-FERPA release
(5)EITHER a SO-2 ACCEPTANCE with a $25 check or cash, OR
SO-2REFUSAL with a copy of your insurance card

Please double check that both the parent and student signatures are on all required forms. Please have athlete bring forms with them on the first day of school OR feel free to drop them off at the school over the summer. Please inform the receptionist to put your forms in Lindley Amarantos' mailbox.
